医药学论文：甘芪益肾冻干粉对肾病综合征模型大鼠的治疗作用

【摘要】 目的 考察甘芪益肾冻干粉对阿霉素型肾病综合征模型大鼠尿蛋白、血浆白蛋白、胆固醇、甘油三酯的影响，观察其对实验性大鼠肾病综合征的治疗作用。方法 用阿霉素制备大鼠肾病综合征模型，对各组大鼠分别给予生理盐水、强的松以及不同剂量甘芪益肾冻干粉，于给药后不同时间测定大鼠24h尿量、总蛋白量，实验结束时测定各组大鼠血浆白蛋白、胆固醇、甘油三酯含量。结果 给予冻干粉5周后各治疗组24h总尿量比模型组明显提高（P＜0.05）；24h尿蛋白量在给药后2、3、4周显著下降（P＜0.05）；其作用强度有明显剂量依赖关系，但低剂量组治疗5周后24h尿蛋白量与模型组无明显差异（P＞0.05）；甘芪益肾冻干粉治疗5周可明显升高模型大鼠血浆白蛋白（P＜0.05），降低胆固醇和甘油三酯水平（P＜0.05）；而强的松在升高模型大鼠血浆白蛋白、降低胆固醇（P＜0.05）的同时，引起甘油三酯水平的显著上升（P＜0.05）。结论 甘芪益肾冻干粉可显著改善实验性肾病综合征大鼠各项生化指标，对肾病综合征有确切的治疗作用。

Therapeutic effect of ganqiyishen powder on rats with experimental nephrotic syndrome
【Abstract】 Objective To evaluate the therapeutic effect of Ganqiyishen powder on rat with experimental nephrotic syndrome according to the levels of urine protein，plasma albumin，cholesterol and triglyceride in pathological modeled animals.Methods Rat's nephrotic syndrome model were prepared with adriamycin infusion through tail vein，saline or prednisone or different dosage of Ganqiyishen powder was administrated to the rats after the nephrotic syndrome model was prepared successfully，total volume of urine excreted in 24h was measured after treated with different therapeutic agents at the 2nd and 5th weekend，and the urine protein level at the 2nd，3rd，4th and 5th weekend was assayed respectively Plasma albumin，cholesterol and triglyceride were analyzed with full automatic blood bio-chem analyzer.Therapeutic effect of Ganqiyishen powder was evaluated based upon the results mentioned above.Results Total 24h urine volume were elevated after treatment，the difference between the powder treated groups and the model group was significant (P＜0.05)，urine protein in powder treated groups were decreased at the 2nd，3rd，4th and 5th weekend compared with those of model group (P＜0.05).Having been treated with Ganqiyishen powder for 5wk，plasma albumin was elevated (P＜0.05) but cholesterol and triglyceride were decreased obviously (P＜0.05).As to the prednisone treated group，both plasma albumin and triglyceride were elevated (P＜0.05)，while the cholesterol was decreased significantly (P＜0.05).Conclusion Ganqiyishen powder could improve the biochemistry index of nephrotic syndrome rats，and the therapeutic effect of the powder on rat's nephrotic syndrome was obvious.

甘芪益肾水煎剂已在临床应用十余年，对肾病综合征（nephrotic syndrome，NS）的临床治愈率高达90%以上，疗效显著。特别有意义的是，其中多数患者为难治性NS，因此是一种有广泛应用前景的潜在NS治疗药物。早期研究证实，甘芪益肾水煎剂的主要有效成分为生物碱类物质，本实验的目的在于考察由总生物碱制备的冻干粉对肾病综合征模型大鼠尿蛋白、血浆白蛋白、胆固醇以及甘油三酯等生化指标的影响，观察其对实验性大鼠NS的治疗作用，为甘芪益肾冻干粉的临床应用提供依据。
1 材料与方法
1.1 药物 醋酸强的松片，广东华南制药厂，规格5mg/片，批号：020716；盐酸阿霉素（Adriamycin，ADR），日本美露香株式会社，规格10mg/支，批号：0109E1；甘芪益肾冻干粉：解放军150医院提供。
1.2 动物 雄性健康SD大鼠，体重170±30g，购自洛阳医学高等专科学校实验动物中心，标准饲料适应环境1周后供实验用。
1.3 NS模型制作 阿霉素型大鼠NS模型参照文献［1］制作。具体方法：ADR7.5mg/kg分2次给予清醒状态下SD大鼠，以尾静脉注射方式给药。实验d1注射ADR 4mg/kg，d8再次注射ADR 3.5mg/kg，对照组用相同方法注射相应剂量生理盐水。
1.4 实验分组及给药 造模大鼠于第2次给予ADR后，随机分为6组，每组10只。分别为：（1）空白对照组；（2）模型组；（3）强的松给药组（20μg/100g）；（4）甘芪益肾冻干粉高剂量组（2.0mg/100g）；（5）甘芪益肾冻干粉中剂量组（1.0mg/100g）；（6）甘芪益肾冻干粉低剂量组（0.5mg/100g）。受试动物造模后，各组按上述剂量每天灌胃给药1次，空白组和模型组给予等量生理盐水，连续给药4周，整个实验过程为5周。
1.5 观察指标及测定方法
1.5.1 一般情况 观察大鼠的精神状况、体重改变、体毛、饮水、活动、水肿情况以及测定24h总尿量。
1.5.2 尿液检查 24h尿蛋白测定：各组大鼠分别于实验第2、3、4、5周置代谢笼中，收集24h尿液，考马斯亮蓝比色法测定尿蛋白量。
1.5.3 血液检查 实验第5周末，在乙醚麻醉状态下，从大鼠眼静脉丛取血，置全自动血液分析仪（CE-LL-DYN3500型，雅培公司）测定血浆白蛋白、总胆固醇、甘油三酯水平。
1.6 数据处理 所有实验数据均以x±s表示，SPSS11.0统计软件处理数据，方差分析比较组间差异，P＜0.05为差异有显著性。
2 结果
2.1 一般情况观察 第1周注射ADR后，除空白对照组外，其余各组动物均有摄食减少、体重下降、腹泻、喜蜷缩、体毛凌乱无光泽等表现；实验第3周，模型组与各治疗组出现不同程度水肿，以四肢水肿为明显症状，模型组2例出现明显腹水；实验第4周，模型组水肿明显，腹水发生率上升，水肿率70%，各给药治疗组水肿程度明显轻于模型组；实验第5周，模型组水肿无明显改善，甘芪益肾冻干粉高、中剂量组及强的松治疗组水肿消除明显。各组造模前以及给药后第2、5周末24h尿量情况见表1。
