医药学论文：慢性活动性乙型肝炎外周血T细胞亚群上活化抗原的表达

摘要:目的 探讨慢性活动性乙型肝炎（CAHB）患者外周血活化抗原CD69、HLA-DR分子在T细胞表达的变化及其意义。 方法 以31例正常人作为对照，采用免疫荧光三色标记流式细胞术，检测30例CAHB患者外周血T细胞亚群及其CD69、HLA-DR抗原表达;同时用荧光定量PCR检测患者的病毒DNA载量。 结果 CAHB患者外周血CD3 + T细胞的百分率（63.78%±9.70%）与正常对照组比较差异有显著性（P< 0.01），CD4 + T细胞的百分率（45.88%±8.51%）明显低于正常对照组（P< 0.001），CD8 + T细胞的百分率（44.64%±7.10%）明显高于正常对照组（P< 0.001）;CD3 + T细胞CD69的表达（6.85%±2.83%）与正常人对照组比较差异也有显著性（P< 0.01），其中CD4 + T细胞CD69的表达（2.31%±0.94%）与正常对照组比较差异无显著（P>0.05），CD8 + T细胞表达CD69水平（3.87%±1.81%）明显高于对照组（P< 0.01）;CD3 + T细胞上HLA-DR的表达（17.09%±4.23%）与正常对照组之间差异有显著性（P< 0.001），其中CD4 + T细胞中HLA-DR + 细胞（7.97%±2.43%）和CD8 + T细胞中HLA-DR + 细胞（5.52%±1.48%）均比正常对照组显著增高（P< 0.01）（P< 0.001）;CD8 + CD69 + 、CD8 + HLA-DR + 细胞的数量与病毒DNA载量之间呈正相关。 结论 CAHB患者存在细胞免疫功能紊乱和淋巴细胞异常激活现象，T淋巴细胞的激活以CD8+T细胞为主，其在抗病毒感染中起着重要作用。

The expression of activated antigens in peripheral blood T lymphocyte subsets in patients with chronic active hepatitis B.
Abstract:Objective To investigate the expression of special marker for activation of peripheral blood T lymphocytes in patients with chronic hepatitis B virus and its significance. Methods Immunofluorescent three-color flow cytometry was used to study the expression of CD69and HLA-DR on T lymphocytes in30patients with chronic active hepatitis B virus and31normal controls.The viral load was determined by fluorescence quantitative PCR. Results Expression of CD3 + T cells were significantly lower in pa-tients（63.78%±9.70%）than that in controls（P< 0.01），meanwhile the expression of CD4 + T cells was45.88%±8.51%，significantly lower than that of the controls（P< 0.001）and the expression of CD8 + T cell was44.64%±7.10%，significantly higher than that of the controls（P< 0.001）.Detection of CD69on CD3 + T cells in patients was6.85%±2.83%，significantly higherthan that of the controls（P< 0.01）.NO significant difference in the expression of CD69on CD4 + T cells was observed be-tween patients and the controls（P>0.05），and the expression of CD69on CD8 + T cells was3.87%±1.81%，significantly dif-ferent from that of the controls（P< 0.01）.The expression of HLA-DR on CD3 + T cells were significance difference between pa-tients（17.09%±4.23%）and that in controls（P< 0.001）.The percentages of CD4 + T cells expressing HLA-DR in patients was7.97%±2.43%，significantly higher than that of the controls（P< 0.01）.Significant difference was observed in the expression of HLA-DR on CD8 + T cells in patients（5.52%±1.48%）as compared with controls（P< 0.001）.There were positive correlations between percentages of CD8 + CD69 + ，CD8 + HLA-DR+cells and viral load. Conclusion The findings support that cellular immunodeficiency exists in patients and that T lymphocytes were abnormally activated in the patients.The activation of peripheral blood T lymphocytes in patients mainly involves CD8+subset and it may play an important role in the immune response to antiviral infection.
