医药学论文：静脉输注脂肪乳剂对成人呼吸窘迫综合征的影响

摘要 脂肪乳剂中长链甘油三酯所含多价不饱和脂肪酸是多种生物活性物质（如前列腺素等）的前体它们在成人呼吸窘迫综合征（ARDS）发病中起着极为重要的作用。长链甘油三酯用于ARDS患者可出现一过性动脉血氧分压下降和肺内分流增加，故应慎重大量使用；中链甘油三酯不含生物活性物质前体，但有较高的生热作用，故宜慢速输注。理论上长链和中链甘油三酯混合输注或调整多价不饱和脂肪酸比例及使用方法，从而调控生物活性物质的产生，可对ARDS患者起一定的治疗作用。

　　Abstract Polyunsaturated fatty acids (PUFA)in logn chain triglycerides (LCT)serve as precursors to the eicosanoids .It is eicosanoids that plays an important role in the pathogenesis of ARDS.LCTcause temporary reduction in arterial oxygen tension and inctease intrapulmonary shunt in patients with ARDS.So large dose of LCTshould be avoided being used in patients with ARDS,Mediym chain triglycerides which not containing precursors have higher themogic effect .Aherefore they should be infused slowly.Theoretically,intravenous fat emulsions might be benificial to patients with ARDSby combining LCT withMCT or modulating the content of PUFA and infusing methods .
